Disulfiram (sold under the trade names Antabuse and Antabus) is a drug used to support the treatment of chronic alcoholism by producing an acute sensitivity to ethanol (drinking alcohol). Disulfiram works by inhibiting the enzyme acetaldehyde dehydrogenase, which means that many of the effects of a "hangover" are felt immediately after alcohol is consumed. "Disulfiram plus alcohol, even small amounts, produce flushing, throbbing in head and neck, throbbing headache, respiratory difficulty, nausea, copious vomiting, sweating, thirst, chest pain, palpitation, dyspnea, hyperventilation, tachycardia, hypotension, syncope, marked uneasiness, weakness, vertigo, blurred vision, and confusion. In severe reactions there may be respiratory depression, cardiovascular collapse, arrhythmias, myocardial infarction, acute congestive heart failure, unconsciousness, convulsions, and death." In the body, alcohol is converted to acetaldehyde, which is then broken down by acetaldehyde dehydrogenase. If the dehydrogenase enzyme is inhibited, acetaldehyde builds up and causes unpleasant effects. Disulfiram should be used in conjunction with counseling and support. Disulfiram has been studied as a possible treatment for cancer Under normal metabolism, alcohol is broken down in the liver by the enzyme alcohol dehydrogenase to acetaldehyde, which is then converted by the enzyme acetaldehyde dehydrogenase to a harmless acetic acid derivative (acetyl coenzyme A). This medication should not be given to a patient without their knowledge. Do not take this medication if you are under the influence of alcohol or have drunk alcohol in the last 12 hours. Show More This medication is used along with counseling and support to treat alcoholism. Disulfiram works by blocking the processing of alcohol in the body. This causes you to have a bad reaction when you drink alcohol. Take this medication by mouth with or without food, usually once daily in the morning or as directed by your doctor. If this medication causes drowsiness, take it at bedtime. Dosage is based on your medical condition and response to therapy. The maximum recommended daily dose is 500 milligrams.

Tolerance to disulfiram doesn’t occur; rather, sensitivity to alcohol increases with longer duration of therapy. Excess acetaldehyde produces a highly unpleasant reaction (nausea and vomiting) to even a small quantity of alcohol. It interacts with ingested alcohol to produce acetaldehyde levels five to ten times higher than are produced by normal alcohol metabolism. Maintenance dosage is 125 to 500 mg daily (average dose 250 mg) until permanent self-control is established. Antialcoholic action: Disulfiram irreversibly inhibits aldehyde dehydrogenase, which prevents the oxidation of alcohol after the acetaldehyde stage. Contraindicated in patients intoxicated by alcohol and within 12 hours of alcohol ingestion; in those with psychoses, myocardial disease, coronary occlusion, or hypersensitivity to disulfiram or to other thiuram derivatives used in pesticides and rubber vulcanization; and in patients receiving metronidazole, paraldehyde, alcohol, or alcohol-containing preparations. Only administer after the patient has abstained from ethanol for at least 12 hr Use ONLY as adjunct to supportive & psychotherapeutic treatment; in motivated patient 500 mg PO q Day initially for 1-2 weeks; not to exceed 500 mg/day Maintenance: 250 mg PO q Day (125-500 mg range); continue therapy until a bases for self-control has been established; patient may continue to take drug therapy for months or even years When Et OH ingested by patient taking disulfiram: flushing, throbbing HA, N/V, diaphoresis, thirst, SOB, syncope, vertigo, blurred vision, confusion; respiratory depression, cardiac arrhythmias, MI or liver failure may occur Use caution in diabetes, hypothyroidism, seizures, nephritis, hepatic impairment Severe hepatitis and/or hepatic failure has been associated with therapy even in patients without prior history of abnormal hepatic function Produces sensitivity to Et OH via blocking its oxidation at acetaldehyde stage, resulting in unpleasant reactions including palpitations, hypotension, chest pain, nausea, vertigo, thirst, flushing, and nausea The above information is provided for general informational and educational purposes only. Our reader Mishelle commented here, “I don’t eat [much] during the day. [I take metformin morning and night.] My blood sugar is still too high in the morning…sometimes 125–140ish.” How can Mishelle’s glucose levels go up if she didn’t eat anything? Her glucose is going up from sources other than digested food. Some of it is produced by the liver from stored starch and fatty acids. Livers that produce too much glucose are one of the main ways diabetes causes high blood glucose levels. Other organs also produce small amounts of glucose. This is called “gluconeogenesis” for you science freaks out there. Organs do this to keep blood glucose from going too low at night or other times of not eating. There is no evidence that tapering the dose after improvement wil prevent a relapse. This usually requires 3 to 10 days of treatment, although it can take longer. It is further recommended that short course, or "burst" therapy, be continued until a child achieves a peak expiratory flow rate of 80% of his or her personal best or symptoms resolve. Anti-inflammatory action: Prednisolone stimulates the synthesis of enzymes needed to decrease the inflammatory response. It suppresses the immune system by reducing activity and volume of the lymphatic system, thus producing lymphocytopenia (primarily of T-lymphocytes), decreasing immunoglobulin and complement levels, decreasing passage of immune complexes through basement membranes, and possibly by depressing reactivity of tissue to antigen-antibody interactions. The mineralocorticoids regulate electrolyte homeostasis by acting renally at the distal tubules to enhance the reabsorption of sodium ions (and thus water) from the tubular fluid into the plasma and enhance the excretion of both potassium and hydrogen ions. Prednisolone is an adrenocorticoid with both glucocorticoid and mineralocorticoid properties. Single-Dose Gene-Replacement Therapy for Spinal Muscular. BECLATE 50 metered dose inhaler - home.
